When you're done, just pop in for a few more signatures*, and you're ready to go!
Before you begin:
Be sure to collect your emergency contact information, your child's doctor & their contact info as well as your child's insurance policy number.

(Note: Our security and safety are of utmost importance, so we have several forms and processes to ensure the best possible care for your kids and everyone who attends Pop In Kids Club. Please allow for extra time during your first visit to complete the registration process.)
